- Document Info: LR Number (auto-generated or manual), Date of creation, and any internal or external Reference number.
- Consignor & Consignee Details: Full Name, Contact, and Address for both sender and receiver, plus GST identification numbers and respective Cities for compliance.
- Dispatch Details: From/To Locations for origin and destination, Truck Number, Driver Name, and Driver Mobile contact.
- Goods Details: Type of Goods description, Total Weight of the consignment, number of Packages, and the declared Value of the goods.
- Freight Charges: Agreed Rate, any Advance payment received, remaining Balance due, and Mode of Payment (cash, online transfer, etc.).
